WHOLE-GENOME CHARACTERIZATION OF NDM-1-PRODUCING Klebsiella pneumoniae ST340 FROM THE ENVIRONMENT

Carbapenem-resistant Enterobacteriaceae strains are classified as priority pathogens by World Health Organization and the presence of carbapenemase-producing Klebsiella pneumoniae strains in the environment causes concern. New Delhi metallo-β-lactamase (NDM) is one of the most common important carbapenem resistance mechanisms and plasmid-mediated blaNDM genes are emerging in hospital settings, but they are uncommon in the environmental sector. In this context, we performed the whole-genome characterization of a blaNDM-1-positive K. pneumoniae strain from surface water. A K. pneumoniae strain, named EW807, was isolated using MacConkey agar supplemented with meropenem (4 mg/L) in February 2020 from Pardo River, Guaíra City, Brazil. Antimicrobial susceptibility testing revealed that EW807 strain was resistant to all antimicrobials tested, except tigecycline, being assigned as extensively drug-resistant (XDR). Whole-genome sequencing was carried out using the Illumina MiSeq platform, and then, bioinformatic analyzes were conducted to determine resistome, plasmidome, virulome, and epidemiologic typing. Strain EW807 belonged to the sequence type (ST) 340 O4:KL15 of the clonal group (CG) 258 and the core genome multilocus sequence typing phylogenetic analysis revealed that EW807 strain was most related to strains from Brazil and the United States. Resistome analysis showed intrinsic and acquired antimicrobial resistance genes (ARGs), highlighting blaNDM-1 and blaCTX-M-15, as well as various mutations in antimicrobial resistance determinants. Two plasmids, pEW807-1 (IncX3) and pEW807-2 (IncFIBK), were identified harboring blaNDM-1 and blaCTX-M-15 genes, respectively. Furthermore, pEW807-2 plasmid carried multidrug resistance and multimetal tolerance regions. Tn3-related sequences were identified in regions harboring blaNDM-1 and blaCTX-M-15 genes. Comparative analysis showed that pEW807-1 and pEW807-2 plasmids were most similar to plasmids from clinical strains from China and Brazil. Virulence genes encoding enterobactin, aerobactin, and type 3 fimbriae were identified. To the best of our knowledge, this is the first report of K. pneumoniae ST340 O4:KL15 carrying blaNDM-1 in the environmental sector. Therefore, these findings evidence an environmental XDR strain of K. pneumoniae, an unusual association of ST340 and blaNDM-1, and the plasmid transmission of blaNDM-1 and blaCTX-M-15 in CG258 and interspecies, alerting to a spread of carbapenemases in the post-pandemic scenario.
